IDPH Office of Health Care Transformation: Who are they and what do they do

The Office of Health Care Transformation is a key point-of-contact for a number of Affordable Care Act related initiatives at IDPH.

•             This training will give an overview of initiatives which include:

- Health Benefit Exchange

- Accountable Care Organizations

- Patient-Centered Medical Home/Health Homes

- Chronic care management initiatives

- Community utility and care coordination

•             The mission of the Office of Health Care Transformation is to promote community
care coordination and advance the patient-centered transformation of the health care system, which should improve care and reduce cost.

Instructors:   Angie Doyle Scar and Abby (McGill) Less, with the Office of Health Care Transformation.

Target Audience:  All IDPH employees
